Application Update

Army JROTC has launched a new instructor application site. New applicants should use the updated application portal below to begin or continue the instructor application process.

Due to recent updates in medical requirements, please follow the steps below to bypass the system prompt for document uploads. Note: Height and weight standards must still be met.

  1. Navigate to the Medical tab.
  2. For Question 1, select “No”.
  3. For Question 2, select “No”.
  4. Under Physical Review, ensure all values are set to 1.

Upload Requirements

  • Photograph: DA-style photo (can be taken with cell phone and uploaded as a PDF) in Army Service Uniform, 4″x6″, ¾ body, taken within 1 year. Include name, rank, and date.
  • Last 3 consecutive official evaluation reports, academic included. Upload as separate files. If missing, include MFR.
  • Most recent Records Brief, ORB/ERB/Soldier Talent Profile.
  • Retirement Orders, Separation Orders, DD 214, Member 4 Copy. NOTE: Form 4187, approved memorandums, and/or retirement letter are not accepted.
  • DA Form 5500, Male, or DA Form 5501, Female, Body Fat Worksheet, dated within 6 months. CAN BE COMPLETED AT ANY ARMY RECRUITING STATION, DOCTORS OFFICE, OR UNIT
  • Official transcripts showing degree awarded, signed by registrar.
  • PHYSICAL, Physical forms are no longer necessary for your application submission. To proceed, please upload a blank document to bypass the prompt.
  • VA RATING, Due to recent updates in medical requirements, please follow the steps below to bypass the system prompt for document uploads. Note: Height and weight standards must still be met.
    1. Navigate to the Medical tab.

ARNG & USAR, Actively Serving

  • DA Form 5016, Chronological Statement of Retirement Points, CAC login required.
  • NGB Form 23A, dated within 90 days.
  • Letter of Recommendation from current Commander.

Interview Process

  • Applicants must complete an interview conducted by a certified interviewer after notification from Cadet Command.
  • Applicant must be in Army Service Uniform.
