Application

Applicants must have a Defense Manpower Data Center (DMDC) Defense Self Service Logon (DS-Logon) “Basic account” to use JQUIP. Those personnel who do not have an account may register to obtain one on the JQUIP login page. Instructions for new applicants requiring a DMDC DS-logon account are available on the website.

The JQUIP portal is scheduled to reopen on Friday, August 1, 2025. We expect a high volume of applications and appreciate your patience as we process them.

Note: When submitting your application, ensure the status displays “Application Submitted.” This confirms successful submission.

Important Information:

  • Incomplete applications will be purged from JQUIP within 30 days of inactivity or 120 days after creation if not submitted.
  • Each document must be uploaded to the correct tab. Incorrect uploads may delay processing or result in return.
  • Waiver requests must be submitted through JQUIP or endorsed by brigade prior to submission.
  • Do not redact, encrypt, or password-protect documents. Protected files will be returned without action.

Before Submitting Your Application

Verify the following documents are uploaded:

  • Photograph:
    • Digital photo (cell phone acceptable) uploaded as PDF.
    • DA-style photo in Army Service Uniform, 4″x6″, ¾ body.
    • Must include: Name, Rank, Date Taken.
    • Photo must be taken within 1 year of application.
  • If applicable – last 3 consecutive official evaluation reports (no gaps, no courtesy copies). Upload as 3 separate attachments. If missing, include a Memorandum for Record (MFR).
  • Most recent Records Brief: ORB (Officer), ERB (NCO), or Soldier Talent Profile.
  • Retirement Orders, Separation Orders, DD 214 (Member 4 Copy – must be final retirement copy).
  • Body Fat Worksheet:
  • Official transcripts from accredited institution (must show degree awarded and registrar’s signature).
  • Most recent physical exam (within 12 months, PHA not accepted):
  • VA Rating Decision (if awarded 30% or greater).
  • Letter of Recommendation from current Commander (must outline cumulative years served, character of service, and current rank held). Signed by first-line commander.

Online Training

Interview

  • Applicants must complete an interview conducted by a certified interviewer after receiving notification from Cadet Command.
  • Applicant must be in Army Service Uniform.
